Financial Agreements - complete ONE for EACH child you have enrolled:

Tuition Agreement (for Kindergarten/1st, Preschool, Enrichment classes)
Lunch ‘N Fun Agreement (needed only if attending Preschool Lunch ‘N Fun)

Emergency Card – Complete and return this form for EACH child you have enrolled

Immunization Form (NOT NEEDED if on file for current year) – Complete and return this fillable or printable form for each NEW child you have enrolled.

One form below can be used for multiple children:

Hospital Release Form – Complete and return this form. One form can be used for multiple children.

Parental Commitment Form – Complete and return this form. One form can be completed for multiple children
