Below are links to all Registration forms. Please print and complete. Notice that some are “per child” and some are “per family”. All forms need to be complete and are due by February 17:

Financial Agreements - complete ONE for EACH child you have enrolled:

Tuition Agreement (for Kindergarten/1st, Preschool, Enrichment classes)
Lunch ‘N Fun Agreement (needed only if attending Preschool Lunch ‘N Fun)

Emergency Card – Complete and return this form for EACH child you have enrolled

Immunization Form (NOT NEEDED if on file for current year) – Complete and return this fillable or printable form for each NEW child you have enrolled.

One form below can be used for multiple children:

Hospital Release Form – Complete and return this form. One form can be used for multiple children.

Parental Commitment Form – Complete and return this form. One form can be completed for multiple children
